How to change the name in ICICI Direct trading and Demat account offline?

To update your name offline, submit the signed Form for Change in Name for Individuals along with a self-attested copy of your PAN card and valid address proof at the nearest Demat-enabled ICICI Bank branch.

Once the change is updated in your Demat account, it will automatically reflect in your ICICI Direct trading account.
